The protagonist Artyom, born before the fire but raised Underground, embarks on a mission to warn mankind of an impending threat. He must traverse from forgotten catacombs beneath the subway to desolate wastelands above where his actions will determine mankind's fate. Along his journey he must also contend with mutant horrors that await outside as well as other Metro Station-Cities struggling for survival amongst each other.
